This warm and welcoming session will explore the spectrum of end-of-life care options, from life sustaining measures to the nuanced topic of medical aid in dying.

This is an hour-long exercise class designed specifically to help keep seniors fit and stay strong enough and flexible enough to age with grace, especially avoiding falls. Meets every Wednesday from 11:00 AM to noon at Vista Crest/Parkside Place. Facilitated by SSG Specialist Sally Hartinian.

Learn effective communication strategies to make your wishes known to your loved ones. SSG specialists, Khari Knight of Willowbridge Solutions and Kamra Viau of Acti-Care In-Home Care, will share real-life case studies to provide a clear understanding of the key elements in advance planning, and will take a deep dive into each of the Wishes as it relates to your individual values and preferences. The “Five Wishes” digital booklet from Aging with Dignity is provided to all participants as a guide.

Join us for a monthly caregiver support group facilitated by SSG Specialist, Blanca Nazareth,LCSW. This meeting is open to anyone in the community who would like to discuss challenges related to caring for aging loved ones as well as discover resources and education regarding aging issues.

Suddenly, individuals and family members find themselves swinging between constantly rotating changed to medications, appointments, transportation coordination, master planning groceries and meals, getting different Dr‘s to communicate with each other.. .and the list goes on! Jill Love, Geriatric Care Manager will explain how care management takes the chaos into calm and helps families navigate these complex season with ease, and how families can do their best to manage these seasons themselves.
